This contains the following necessary forms(Emergency Medical, Insurance, Cognitive Testing consent, and Drug and Alcohol testing consent) required for athletes to complete prior to participation in practice or games.

This contains the Preparticipation Physical Evaluation Form(which needs to be signed by a medical professional) as well the Eligibility and Authorization Statement, Concussion Information, and Sudden Cardiac Arrest/ Lindsey's Law Form.
